Opportunity Partners is currently hiring a Designated Coordinator to join their team!

The purpose of this position is to plan, manage and implement individualized service programs for people choosing services from Opportunity Partners, and to provide direction to those people in conjunction with the total program objectives.  The incumbent also manages annual participant assessments and reviews and determines a course of action, and assures compliance with regulatory reporting requirement.

Salary & Hours: $14.00-15.00/hr (DOQ); Mon-Fri; Full-time; NO EVENINGS OR WEEKENDS!

Responsibilities:
  • Plan, manage and implement individualized service programs for people choosing services from Opportunity Partners
  • Assist these individuals throughout the day, including supporting throughout the building, helping during lunch and break times, and assisting with personal cares and med administration
  • Provide supervision and assistance to the individuals that we support on the production floor
  • Plan and implement lessons and activities that will help individuals learn and practice independence and integration in the community
  • Provide leadership, counseling, and guidance that will assist individuals in reaching their goals
  • Demonstrate teamwork and collaboration with co-workers and team members
DUTIES AND RESPONSIBILITIES MAY BE ADDED, DELETED, OR CHANGED AT ANY TIME
AT THE DISCRETION OF MANAGEMENT.
 
Essential Requirements:
Designated Coordinator status is required, and must be met through one of the following:
  1. A bachelor’s degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
  2. An Associate’s degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
  3. A diploma in a field related to human services from an accredited post-secondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  4. a minimum of 50 hours of education and training related to human services and disabilities AND
  5. 4 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Knowledge, Skills and Abilities
  • Knowledge of disability populations, positive supports, and person centered planning
  • Ability to organize, plan, prioritize work
  • Verbal and written communication skills
  • Organized
  • Ability to recognize and develop work site procedures
  • Ability to analyze jobs in relation to people served abilities
  • Ability to react quickly in emergency situations
  • Proficient in Microsoft Office programs (Work, Outlook, and Excel)
  • Ability to develop ideas and innovative approaches
  • Ability to maintain a positive and professional rapport with a diverse group of people
